If you would like to enter into this arrangement you will need to complete the Salary Sacrifice Agreement Form, then return to <a href=\"mailto:benefits@alteryx.com\">benefits@alteryx.com<\/a>. Agreements are effective from July 1 \u2013 June 31, and must be renewed July 1st of every year.<\/td>\n<\/tr>\n<tr class=\"rowA\">\n<td width=\"229\">Healthcare<\/td>\n<td width=\"473\">Exclusive Top Level Corporate Hospital &amp; Extras plan \u2013 Alteryx pays 100% of employee cost for self and dependents. A qualifying dependent is a spouse, partner and dependent child(ren).<\/td>\n<\/tr>\n<tr class=\"rowA\">\n<td width=\"229\">Group Life &amp; TPD<\/td>\n<td width=\"473\">Provides 3x\u2019s annual base salary benefit in the event of the employees death or total and permanent disablement. Employees are auto-enrolled and it\u2019s 100% covered by the company.<\/td>\n<\/tr>\n<tr class=\"border_bottom\">\n<td width=\"229\">Group Income Protection<\/td>\n<td width=\"473\">Benefit is 75% base salary (after a 90 day waiting period), up to age 65 in case of sickness or injury that prevents employees from working. Employees are auto-enrolled and it\u2019s 100% covered by the company.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<h3>New Hires\/Joiners<\/h3>\n<p>New hires must enroll within 30 days of your start date. If you miss your initial enrollment period, you will be able to join during annual renewal in April of every year. Our local broker will be contacting you to schedule a meeting to run through benefits and enrollment.<\/p>\n<h3>Dependents<\/h3>\n<p>If you experience a life event, such as marriage, divorce, or birth of a child, you may add or remove dependents from your plan. Please email the Benefits Team right away so that we can make the change. You have 30 days from the date of the event to make any changes. Here are some examples:<\/p>\n<p><strong>Prioritize your physical and mental health.<\/strong><br \/>\nBy getting plenty of sleep, incorporating regular exercise, and healthy eating habits, you not only are taking care of your physical health but your mental health as well. Try home workouts from YouTube, and schedule video therapy appointments to keep your body and mind healthy<\/p>\n<p><strong>Take regular work breaks.<\/strong><br \/>\nMake sure to schedule time away from your desk to get fresh air and sunlight.<\/p>\n<p><strong>Work at staying connected.<\/strong><br \/>\nFaceTime or call family, friends and colleagues to catch up and visit (virtually through video when possible).<\/p>\n<p><strong>Use your free time to relax.<\/strong><br \/>\nRelax with reading, music, a fun hobby, or meditation \u2013 the Calm app is a great resource. Listen to calming sounds or music \u2013 YouTube has a library of nature and meditation content to stream in the background during work time and kids school time. Try searching for \u201cocean waves crashing\u201d, \u201cnature\u201d, \u201cspace scenery\u201d, and \u201cmeditation frequency music\u201d.<\/p>\n<p><strong>Focus on things in your life that are going well and that you can control.<\/strong><br \/>\nShifting your focus on the things that are going well in your life can give you a sense of control in challenging times. Try writing down three things you are grateful for everyday.<\/p>\n<p><strong>Limit how much time you spend reading or watching the news.<\/strong><br \/>\nWatching too much news and\/or TV can lead to anxiety, depression and increased stress according to the American Psychological Association. Stick to factual sites such as the World Health Organization (WHO) at\u00a0<a href=\"https:\/\/www.who.int\/\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/www.who.int\/<\/a>\u00a0or the Center of Disease Control (CDC) at\u00a0\u00a0<a href=\"https:\/\/www.cdc.gov\/\" target=\"_blank\" rel=\"noopener noreferrer\">https:\/\/www.cdc.gov\/<\/a>.<\/p>\n<p><strong>Reach out for help.<\/strong><br \/>\nIf feelings of anxiety still arise, it is okay to reach out for help. We will all be on video conference meetings as well, so you will be prepared when you need to turn that camera on.<\/li>\n\t<li><strong>If possible, have a dedicated \u201coffice\u201d space to work<\/strong><br \/>\nEnsure that it\u2019s a comfortable space \u2013 use a comfortable chair, place your monitor at the correct height, and plug your keyboard and mouse into your laptop. You will be less productive and will fatigue if you type on the laptop keyboard and use the laptop\u2019s trackpad all day. A proper set up with good lighting is key to keeping you in a productive and healthy mental state. See below the Human Scale Flyer provided for proper ergonomic setup at home.<br \/>\nIt is also important to have space that you can physically and mentally associate with work, a space you can enter and leave each day. This will help you create a boundary between work and personal time so you don\u2019t fall into the habit of \u201cI\u2019ll just do that one more thing before I stop working\u201d. Once work is done, leave your work space. Close the door to the room if possible. For those of us who are working and living in the same space, do something at the end of the day to signal to yourself that work is finished. Close your laptop and turn off your monitor.\u00a0 If you find yourself continually going back to the laptop for \u201cone more thing\u201d in the evenings, throw a blanket over your setup so you can\u2019t see it.<\/li>\n\t<li><strong>Reach out to colleagues \u2013 use video and phone over email<\/strong><br \/>\nIf you gain energy and excitement from being in the office with colleagues, and find yourself missing that connection, take the initiative to schedule collaboration video calls with your cameras so you can see each other. And pick up the phone often to have a conversation versus sending an email.<\/li>\n\t<li><strong>Take regular breaks<\/strong><br \/>\nTake a full lunch break without work distractions. Disconnect and walk away from your work setup.\u00a0 Get outside if possible, read a book, call a friend or watch some funny videos. Disconnecting during breaks, and before and after work hours, will help you maintain more structure in your day.<br \/>\nBuild in breaks in your daily schedule to refresh. Spend 10 minutes listening to some music or doing a few stretches before getting back online.\u00a0 It\u2019s helpful to block off time on your calendar so people are aware of the times you aren\u2019t available and, more importantly, so you have reminders when to take that break.<\/li>\n\t<li><strong>If you\u2019re feeling stressed\u2026<\/strong><br \/>\nGive your brain a nice mental health break during the day by standing up regularly, looking outside at nature, or taking a couple deep breathes before reading that next email.
