Life, Accidental Death and Dismemberment (AD&D), and Disability insurance provide financial security to you and your family if you pass away, become seriously injured, or experience an illness or injury that prevents you from working.
As an employee, you automatically receive Basic Life and AD&D coverage in the amount of 2 times your base salary up to a maximum of $500,000. This coverage is provided at no cost to you.
You may choose anyone to be the beneficiary of your Life and AD&D policies. Review your beneficiary designation periodically to make sure it reflects your current wishes.
In addition to Basic Life and AD&D, you may purchase supplemental Life and AD&D coverage at discounted rates. The chart below describes the amount of supplemental life coverage you can buy for yourself, your spouse, and your child(ren).

What Is EOI? Evidence of Insurability (EOI) is the process of providing health information to qualify for certain types of insurance coverage. If you elect supplemental Life and AD&D coverage above the guaranteed issue limit or after the guaranteed issue period, you will be required to submit a health questionnaire (in some cases, a physical exam may be required). Your questionnaire will be reviewed by Unum and you will be notified of their decision directly.
California employees are covered under the California State Disability Insurance (SDI) program. The SDI program covers up to 70% of your salary with a maximum weekly benefit of $1,540 for up to 52 weeks.
The STD plan provides 70% of your weekly salary, to a maximum of $1,357 per week for up to 25 weeks of a disability. Benefits begin after 7 days of missed work because of an accident or an illness. You are automatically covered under the STD plan at no cost to you.

LTD coverage is important because anyone at any age may become injured or ill for an extended period of time. LTD protects you from loss of income in the event you are unable to work due to an illness, injury, or accident for a period of time that exceeds the short-term disability benefit. You are automatically covered under the LTD plan at no cost to you.
LTD coverage will replace 60% of your base salary to a monthly maximum of $10,000 if you are disabled for more than 180 days and are unable to work. LTD benefits are offset by other sources of income, such as Social Security and workers’ compensation.
